Tirzepatide is a synthetic peptide with dual agonist activity at the glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptors, both class B G-protein-coupled receptors. It was engineered from the native GIP sequence. Mechanism: combined GIP and GLP-1 receptor activation is associated with insulin secretion, glucoregulation, gastric emptying, and appetite regulation. Tirzepatide is the active ingredient in the FDA-approved pharmaceutical products Mounjaro (type 2 diabetes) and Zepbound (chronic weight management), approved by the U.S. FDA in 2022 and 2023 respectively. Human clinical evidence: phase 3 programs (SURPASS for type 2 diabetes; SURMOUNT-1 for obesity, NEJM 2022) reported substantial, dose-dependent weight reduction and glycemic improvement.
